Extracting produces one new document containing just the pages you named. Splitting breaks a document into several files. If you only want a chapter or a single form out of a long report, extraction is what you want.
Yes. A selection like 1, 4, 9-11 is fine — the pages are collected into one new document in ascending order.
Page content and annotations anchored to the extracted pages are copied across. Document-level features such as bookmarks and the interactive form catalogue belong to the original file and are not carried over. If you need those preserved, delete the unwanted pages instead of extracting.
